transfer your prescription

To transfer your current prescription to SMP Pharmacy, simply submit the transfer form below and one of our skilled team of pharmacists will then take the following steps:

  • We will contact your current pharmacy
  • Process patient’s medications through their insurance
  • Prepare patient’s medication for pick-up or next day delivery
